During this webinar, RN Project Coordinators from the Home Health Quality Improvement (HHQI) National Campaign team will provide an overview of Diabetes Best Practice Intervention Package (BPIP) including improvement strategies, tools and resources. Process for accessing the HHQI University courses of ‘Diabetes Self-Management’ & ‘Diabetes and Your Patients’ (a course for home health aides) will be shared.

During this webinar, RN Project Coordinators from the Home Health Quality Improvement (HHQI) National Campaign team will provide an overview of the Disease Management: Heart Failure Best Practice Intervention Package (BPIP) including improvement strategies, tools and resources. Highlights will include HHQI University courses presented by nationally known speakers titled ‘Exercise Prescription for the Home Health Patient with Heart Failure’ & ‘Maintenance Therapy for Home Health Patients with Heart Failure.’

Cardiovascular Disease beyond Heart Failure

During this webinar, RN Project Coordinators from the Home Health Quality Improvement (HHQI) National Campaign team will provide an overview of the Preventative Cardiovascular (Aspirin as Appropriate, Blood Pressure Control, Statin Therapy, and Smoking Cessation) BPIPs including improvement strategies, tools and resources. Additional highlights will include the HHCDR (Home Health Cardiovascular Data Registry) and the multiple HHQI University courses available, including a home health aide course, ‘Heart Attack & Stroke Risk Factors, Signs & Symptoms.

Respiratory Disease

Highlights will include discussion of tobacco cessation resources, immunization resources, and unique patient self-management resources.

Sharing of registration or fees with other agencies and/or individuals is prohibited. If your agency purchased only one phone/webinar connection but connects to the presentation from multiple site areas, your agency will be invoiced for the additional lines used to participate.

 

Presenters:

Misty Kevech, RN, MS is an RN Project Coordinator for Quality Insights, the Quality Innovation Network (QIN)-Quality Improvement Organization (QIO) for the states of West Virginia, Pennsylvania, Delaware, New Jersey, and Louisiana. Her responsibilities are with the Centers for Medicare & Medicaid Services’ (CMS) Home Health Quality Improvement (HHQI) National Campaign and involve researching and writing Best Practice Intervention Packages (BPIPs) for home health. BPIPs provide information, tools, and resources on key topics to improve patient outcomes. One of Misty’s focus areas is on underserved populations and their specific needs including patients who are in underserved areas, those who are dual-eligible, experience significant health disparities, and those served by small home health agencies.

Cindy Sun, MSN, RN is the Lead Cardiovascular Project Coordinator for Quality Innovation Network (QIN)-Quality Improvement Organization (QIO) for the states of West Virginia, Pennsylvania, Delaware, New Jersey, and Louisiana. Her responsibilities are with the Centers for Medicare & Medicaid Services’ (CMS) Home Health Quality Improvement (HHQI) National Campaign since 2009. Her primary responsibilities include organizing the 100+ Network Coordinators, assisting home health agencies with integrating data-driven quality improvement into their QI plans, and assisting with the integration of preventative cardiovascular care into the home health setting through the development of educational resources and the Home Health Cardiovascular Data Registry.

 

Continued Education Credits:

This program has been designed to meet the continuing education requirements for the Minnesota Board of Nursing for contact hours.

Prior to each webinar, a GoToWebinar link will be e-mailed to you. You will need to register through this link to attend the webinars. You will be sent a final confirmation email which will include a join link to access the webinar, a dial-in number and an access code/PIN to listen in via telephone. Google Chrome may be the optimal browser to use. Do not forget to enter your PIN when you call in. You will also be sent any pertinent handouts if available, the sign-in sheet, and evaluation form. If your Contact Person does not receive the email confirmation prior to the webinar, please contact our office to verify your registration.

RECORDING: You will be sent the link to the webinar recording following the presentation.

 

Cancellation Policy

Payment must be made in advance of the program. Due to the expense of using the telephone/web hook-up, Webinar registrations may not be shared between agencies. Your registration covers the access of only one log in to the webinar. Handouts and related materials will be sent to you shortly before the program to the email address you provide. Refunds will be issued for those that cancel up to four (4) business days prior to the program less a $25 cancellation fee. Cancellations made less than 4 business days, or ‘no shows’ will forfeit the registration fee. Cancellations must be received in writing via e-mail.
