Home health agencies are measured on a variety of process and outcome performance measures. CMS reports this performance to the Home Health Compare website where patients can choose their preferred agency based on these quality and performance measures. Agencies are required to report multiple measures and CMS summarizes nine of these measures into single star rating measures. The MHCA Clinical Quality team has come together to offer individualized education on each of these nine areas to help enhance the star scores across the state. Jani Helm, RN, Director of Clinical Care, Knute Nelson Home Care
As an RN and Director of Clinical Care at Knute Nelson Home Care, Jani applies her competencies in nursing to ensure the highest quality of clinical services are being provided to Knute Nelson Home Care patients, while leading a team of skilled professionals. Her leadership in nursing spans nearly eight years of experience. Jani holds an Associate of Science Degree in Nursing from Minnesota State Community and Technical College, and her Bachelor of Science Degree in Nursing from Rasmussen. She serves as chair for the Minnesota Home Care Association Clinical Quality Team and has also participated in 2014-2015’s Alexandria Area Chamber of Commerce Leadership Program. Additionally, Jani is a volunteer nurse for Koenen’s Country Preschool.
Mary Jane Fraser, PT, Quality/Compliance Manager, Allina Health Home Health
Mary has worked in Home Health for the past 24 years. She has held variety of roles including staff PT, PT Case Manager, Utilization/Compliance Coordinator, Clinical Manager and currently Quality/Compliance Manager for Allina Health Home Health. Mary graduated from the College of St Scholastica with a degree in Physical Therapy. She worked in an acute care regional trauma center and an inpatient rehabilitation unit before transitioning to Home Health Care. Mary is a member of the Minnesota Home Care Association, American Physical Therapy Association Home Health Section, and attended the Delta National Excellence in Therapy Forum as a representative of the Minnesota Home Care Association. Mary has presented at many Minnesota Home Care Association Annual Conferences as well as the National Home Care Association conference in 2010.
Kristy Husen, PT, Director, CentraCare Health Home Care & Hospice
Kristy is the Director at CentraCare Health Home Care & Hospice since 2014 and prior to that served as the Rehab Coordinator (since 2008). She has been a Physical Therapist since 2001 obtaining her Masters of Physical Therapy (MPT) from The College of St. Scholastica. She has served on the MHCA Rehabilitation Team from 2009-2015, transitioning to the Clinical Quality team this year. Kristy has presented as a part of the Rehab Team and at other conferences throughout her career.
