Be Prepared for Your Survey – New MDH Tools to Help!

The Minnesota Department of Health (MDH), Home Care and Assisted Living Program (HCALP) has a new section on its website where providers will find new and updated survey forms. MDH uses these forms to conduct surveys of temporary licensed and licensed home care providers. Providers should find the forms helpful to understand state requirements and can use the forms to conduct self-audits.

HCALP encourages providers to use the forms to prepare for MDH license surveys. Applicants for home care licensure will also want to use the forms to learn about requirements and understand the survey process.

A note for Medicare certified home health agencies (HHAs):
HHAs should refer to Medicare requirements. These new state licensing forms do not apply to HHAs.
