- Understanding medications and treatments to prevent hospitalizations.
- Learn strategies to keep CHF patients at home and healthy.
Faculty: Josaleen Davis, MD, Dr. Davis is a board-certified internal medicine and geriatric medicine physician with professional interests in falls, preventive medicine, dementia and medication interactions (pharmacology). Dr. Davis graduated from the University of Minnesota Medical School in 2007. She did residency and fellowship at Maine Medical Center in Portland Maine. She works at Allina Senior Health Department managing the primary care home visit program. She has 3 school aged girls and 2 geriatric pets; a pug named Murray and cat named Peaches.
| Part 2: Chronic Disease Management: Diabetes Recorded on Thursday, July 21, 2022 Objectives: - Review screening and prevention guidelines for diabetes and impacts for home care.
- Understand general treatment algorithms depending on age, function, and goals.
- Provide strategies for assisting patients to stay healthy and at home.
- Address ways to prevent hospitalization related to diabetes.
Faculty: Ben Rosenstein, MD, MA, completed his family medicine training at the University of Minnesota St. John's Hospital Family Medicine Residency Program. Following residency, he pursued his interests in the care for older adults and completed a Geriatrics Medicine Fellowship at the University of Wisconsin School of Medicine and Public Health. Additionally, he has completed a Master's in Bioethics at the University of Minnesota Center for Bioethics. His thesis focused on care at home for complex older adults. He has deep interest in primary and specialty care of older adults in multiple settings as well as health and aging policy initiatives.
| Part 3: Chronic Disease Management: Chronic Obstructive Pulmonary Disease (COPD)
Recorded on Wednesday, July 27, 2022 Objectives: - Recognize treatment for COPD along with prevention of acute COPD exacerbations.
- Recognize symptoms of COPD exacerbation.
- Review treatment of COPD exacerbation, with a focus on approaches to reduce hospitalization and keep patients at home.
Faculty: Sarah Markert, MD, joined Allina Health in 2006 as a family physician at the Hastings Clinic. In March of 2020 she joined Allina’s Complex Care for Seniors Program(CCSP) – which provides primary care in the home for complex, frail elderly. Dr. Markert left the clinic after 15 years in March 2022 to focus on providing care with CCSP along with Hospice. She has also worked a couple years at the Prescott nursing home and has been the Medical Director of Hastings Senior Health and Living (formerly Augustana) for the past few years. She lives in River Falls, WI with her husband, 3 children and dog Chloe.
