OASIS E is live, but the new questions are not yet part of HHVBP. This session will refocus on those OASIS items included in HHVBP; we will review guidance on the OASIS items included in HHVBP and steps you can take so your patients are improving in these metrics as often as possible.

Identify where your customer service skills need to be improved. We will review the specific HHCAHPS questions that contribute to this measure and provide strategies to use to improve in these key areas.

Part 4: Identify Weaknesses and Develop Strengths

Learn how to interpret your data to identify those areas in which you need to focus your improvement efforts. This session will review an action plan that can be used to improve any of the HHVBP measures. We will also review the roles of the improvement team and provide examples of who could fill each role.

Charles Breznicky Jr., RN, MSN, MBA, HCS-D, Director, SimiTree

As a Consulting Director for SimiTree, Charlie Breznicky is a leading expert in clinical operation and VBP. With more than 20 years of clinical field experience, Charlie has assisted agencies in developing QAPI Programs, preparing for surveys, succeeding under the Review Choice Demonstration, adapting to the changes to the Home Health Conditions of Participation and the Patient Driven Groupings Model, and preparing for Value Based Purchasing. He obtained dual master's degrees in nursing administration and in business administration from LaSalle University in 2014 and obtained his HCS-D certification in 2021.

Jennifer D. Osburn, RN, HCS-D, COS-C, Director of Education, Home Health Solutions

As a Senior Manager for SimiTree, Jennifer Osburn is a well-versed, nationally known home health educator. Having a diverse background that includes extensive field staff, administration, and technology/software experience allows opportunities for her to establish a unique, “real-world” connection with home health professionals of all disciplines. Jennifer’s 29 years of clinical experience, combined with an ability to engage her audience, has led to numerous speaking engagement invitations across the nation. She holds specialist certifications for Home Health Diagnosis Coding and OASIS. 

Michael Simione, MBA, Director, Cost Reports

As the Director of Cost Reporting for SimiTree, Michael Simione heads a team of financial experts whose work leads the nation each year in the number of cost reports prepared for Medicare-certified home health agencies. A leading expert in PDGM and VBP, Simione has wide-ranging experience in budgeting, financial and operational consulting, strategic planning, data analytics, and benchmarking. He holds an MBA from Quinnipiac University and was recognized as the top graduate in his class.
